Thanks, here is the file if you want to use it. It is 4.25" x 4.25". For the image, I simply printed the image on tissue paper, cut to size and used mod podge to adhere to tile. The tiles were on clearance at Lowes for 34¢ each.

@CherylC - Tissue paper like used for gifts and gift bags? Do you have to have a special printer, special ink, etc? Will you come on over and teach me how to tie a pretty bow? I have several spools of ribbons, but can't make a good bow.
@nomem, yes regular gift tissue paper from the Dollar Tree. What I do is cut to typing paper size (8.5 x 11), tape to a sheet of paper to give it stability. Then I run it through my regular Epson WorkForce 545 printer on regular setting. While still taped to the paper I cut to size. Because the paper is so soft, I brush a light coat of Mod Podge onto the tile, then "slide" the image onto the tile, carefully smoothing the image (really delicate paper) lightly with my finger removing any bubbles (if you have a brayer this is the ideal project). Let dry for about 20 minutes, then brush one coat of Mod Podge (Matte) on top of image going in one direction, let dry for about 20 minutes, then brush another coat in the same direction, repeat drying time, then brush another coat in the opposite direction (2 coats in horizontal and 2 coats vertical), repeat drying time. Once it is dry, I make a final coat of Mod Podge (Gloss). I cut down a piece of felt to size and glue to bottom of tiles. They fit perfectly in the box. The tile size is 4" x 4".
Tip: Wire ribbon is your friend.

Thanks for the file, love box files. I changed the file name to include Cheryl like I do all files, so I can remember to thank if I ever get around to cutting anything. This I am pretty sure I can use because I have a lot of 4x4 tiles laying around here.

@CherylC Your gift is very pretty and sure to be a treasured keepsake of this special time in their lives. Beautiful bow, too. How do you make a double bow like that? When it comes to making bows.. am all thumbs. Would you please share your method?
I got some of the 31 cent tiles and went to town. I didn't print on tissue paper, but printed on tracing paper and ended up with a mess. I ended up printing on regular printer paper and gluing them on. I posted some pictures on my bloggie if you want to see.
I still need to make the box to put them in, but I'm trying to get some "keepers" first. :)

@CherylC - what type of tiles were these? Were they the "tumbled " ones with lots of pits, or another type? They look great! and thanks for the box, it will come in handy.
They were the "tumbled" ones. Those work better for me with mod podge
